We Are a Medi-Cal (CalAim) Provider

Omatochi is now an approved provider for Medi-Cal, specifically under 
the California Advancing and Innovating Medi-Cal (CalAim) initiative.  

Community Support addresses health-related social needs, helping members lead healthier lives and steering clear of higher, costlier levels of care. This includes:

Transition to Assisted Living Facilities​

Omatochi’s program facilitates the transition of individuals to Assisted Living Facilities, promoting community living over institutional care. This initiative supports individuals moving from nursing facilities back into home-like, community settings or prevents nursing facility admissions for those with imminent needs. We aim to enhance the quality of life by fostering independence and community engagement.

Transition to Home​

Our Community Transition Services/Nursing Facility Transition to a Home initiative assists individuals in avoiding further institutionalization. By providing non-recurring set-up expenses, we empower individuals to transition from licensed facilities to living arrangements in private residences. This program emphasizes the importance of personalized care within a familiar and comfortable environment.

Home Modifications​

Environmental Accessibility Adaptations ensure individuals can thrive in their homes by making necessary physical adaptations. These modifications are crucial for ensuring health, welfare, and safety, ultimately fostering independence and a sense of security within the home.
