In-Person Care Management Where They Live​

For high-need members, personalized care should be where they call home. ECM goes beyond traditional healthcare models by providing eligible members with a dedicated Lead Care Manager. This ensures comprehensive care management right in the member’s living space, offering support that resonates with individual needs.

Enhanced Care Management: Elevating Healthcare for High-Need Individuals

Omatochi proudly introduces Enhanced Care Management (ECM), a comprehensive Medi-Cal managed care benefit designed to address both clinical and non-clinical needs of high-need individuals. ECM achieves this through the coordination of services and the implementation of comprehensive care management. It is important to note that ECM services can be provided simultaneously with Community Support, offering a holistic approach to healthcare.

Community Support: Enhancing Lives Through Comprehensive Care

Omatochi provides a spectrum of community-based services under the Community Supports program, addressing health-related social needs and offering viable alternatives to hospital care, nursing facility admissions, emergency department visits, and other costly services.
