Transforming Medi-Cal: ECM and Community Support Revolutionizing Healthcare
At the heart of the Medi-Cal transformation, Enhanced Care Management (ECM) and Community Support stand as transformative pillars, reshaping the landscape of healthcare by breaking down traditional barriers and extending care beyond hospital walls into the heart of our communities. This innovative approach introduces a more coordinated and personalized care system, providing in-person care management where it matters most - right where Californians live.
ECM and Community Support redefine healthcare by breaking free from traditional confines. No longer confined to hospitals and healthcare settings, these programs bridge the gap between clinical care and community well-being, ensuring a comprehensive and accessible healthcare experience.
Introducing a Better Way to Coordinate Care:
Coordination is critical to adequate healthcare. ECM and Community Support usher in a better way to coordinate care by connecting various aspects of a member's health journey. This ensures that quality care is seamlessly integrated, whether at a doctor's office, a dental clinic, in collaboration with a social worker, or within the warm embrace of a community center.
In-Person Care Management Where They Live:
For high-need members, personalized care should be where they call home. ECM goes beyond traditional healthcare models by providing eligible members with a dedicated Lead Care Manager. This ensures comprehensive care management right in the member's living space, offering support that resonates with individual needs.
What This Change Means for Californians:
ECM brings tangible benefits for the people of California. ECM, a new statewide Medi-Cal benefit, offers eligible members with complex needs:
Single Lead Care Manager: Access to a dedicated professional orchestrating comprehensive care management.
Quality Care Everywhere: Connections to quality care, no matter where members seek it – be it a medical office, a dental facility, with a social worker, or at a community center.
Community Support addresses health-related social needs, helping members lead healthier lives and steering clear of higher, costlier levels of care. This includes:
Assistance securing and maintaining stable housing.
Medically Tailored Meals:
Access to specially designed meals supporting short-term recovery.
Diverse Community-Based Services:
A variety of community-based services catering to individual needs.
As we embark on this transformative journey, ECM and Community Supports mark a commitment to building a healthier and more connected California. For more information on eligibility and accessing these innovative programs, contact us at 925-905-1662.
Omatochi Enhanced Care Management: Elevating Healthcare for High-Need Individuals
Omatochi proudly introduces Enhanced Care Management (ECM), a comprehensive Medi-Cal managed care benefit designed to address both clinical and non-clinical needs of high-need individuals. ECM achieves this through the coordination of services and the implementation of comprehensive care management. It is important to note that ECM services can be provided simultaneously with Community Support, offering a holistic approach to healthcare.
Enhanced Care Management
Under the Enhanced Care Management program, Omatochi is committed to providing exemplary services to the following population of focus:
Adults Living in the Community at Risk for LTC Institutionalization: We focus on individuals at risk of long-term care (LTC) institutionalization who require lower-acuity skilled nursing. This includes those experiencing challenges with activities of daily living (ADLs), communication difficulties, access to food, stable housing, living alone, and the need for conservatorship or guided decision-making.
Our goal is to provide continuous community residence with wraparound support, ensuring a safe and fulfilling life.
Safe Transition into the Community: Our ECM program offers intensive care coordination for nursing facility residents interested in moving out of the institution. This service facilitates a safe and seamless transition into the community.
By leveraging ECM, we empower individuals to reside continuously in the community, fostering independence and improved overall well-being.
At Omatochi, our Enhanced Care Management program is more than just a service; it's a commitment to improving the lives of high-need individuals. We understand that healthcare is not one-size-fits-all, and our approach is personalized, ensuring that everyone receives the care and support they need to thrive.
Omatochi Community Support: Enhancing Lives Through Comprehensive Care
Omatochi provides a spectrum of community-based services under the Community Supports program, addressing health-related social needs and offering viable alternatives to hospital care, nursing facility admissions, emergency department visits, and other costly services.
Omatochi's program facilitates the transition of individuals to Assisted Living Facilities, promoting community living over institutional care. This initiative supports individuals moving from nursing facilities back into home-like, community settings or prevents nursing facility admissions for those with imminent needs. We aim to enhance the quality of life by fostering independence and community engagement.
Our Community Transition Services/Nursing Facility Transition to a Home initiative assists individuals in avoiding further institutionalization. By providing non-recurring set-up expenses, we empower individuals to transition from licensed facilities to living arrangements in private residences. This program emphasizes the importance of personalized care within a familiar and comfortable environment.
Environmental Accessibility Adaptations ensure individuals can thrive in their homes by making necessary physical adaptations. These modifications are crucial for ensuring health, welfare, and safety, ultimately fostering independence and a sense of security within the home.
Our Personal Care program is designed to support individuals who need assistance with various daily activities. From bathing and dressing to personal hygiene, cooking, and eating, we prioritize individualized care plans that enhance autonomy and quality of life.
Omatochi's Homemaker Services offers comprehensive support for seniors on their aging journey. From assistance with self-care to meal preparation, light housekeeping, and companionship, we aim to foster a sense of well-being and connection. Our services extend beyond the practical to provide meaningful social engagement.
Omatochi recognizes the invaluable role of caregivers. Our Respite Services offer short-term relief to caregivers, providing temporary supervision for those requiring occasional support. This allows caregivers to recharge, knowing their loved ones are in capable hands, fostering a supportive and sustainable caregiving environment.
Omatochi is committed to creating a community where individuals can thrive, regardless of their unique needs. Our Community Supports program is a testament to our dedication to delivering holistic care beyond traditional healthcare, promoting independence, dignity, and the highest quality of life for all.
