ECM achieves this through the coordination of services and the implementation of comprehensive care management. It is important to note that ECM services can be provided simultaneously with Community Support, offering a holistic approach to healthcare.

Under the Enhanced Care Management program, Omatochi is committed to providing exemplary services to the following population of focus:

Community Residents
Adults Living in the Community at Risk for LTC Institutionalization: We focus on individuals at risk of long-term care (LTC) institutionalization who require lower-acuity skilled nursing. This includes those experiencing challenges with activities of daily living (ADLs), communication difficulties, access to food, stable housing, living alone, and the need for conservatorship or guided decision-making.
Our goal is to provide continuous community residence with wraparound support, ensuring a safe and fulfilling life.

Nursing Facility Residents
Safe Transition into the Community: Our ECM program offers intensive care coordination for nursing facility residents interested in moving out of the institution. This service facilitates a safe and seamless transition into the community.
By leveraging ECM, we empower individuals to reside continuously in the community, fostering independence and improved overall well-being.
