Residents
Adults Living in the Community at Risk for LTC Institutionalization: We focus on individuals at risk of long-term care (LTC) institutionalization who require lower-acuity skilled nursing. This includes those experiencing challenges with activities of daily living (ADLs), communication difficulties, access to food, stable housing, living alone, and the need for conservatorship or guided decision-making.
Our goal is to provide continuous community residence with wraparound support, ensuring a safe and fulfilling life.
